The forms needed for health care claims and to set up direct deposit of payroll checks are provided, as well as a claim forms for flexible spending accounts in which City employees are enrolled. In addition, official copies of the City Holidays, the City Ordinance Amendments to Personnel Policies and Procedures and the City Personnel Policies and Procedures Manual can be downloaded.
